Logicalis CCPA Consumer Information Request Form

Pursuant to the CCPA, you or your representative may make a personal information request twice in a 12-month period. 
Each type of personal information request available to you is listed below. In order to process an information request, Logicalis may need to collect additional information from you or your representative so that it can verify your identity.

Please note that Logicalis may not be able to process any request that fails to provide sufficient information. We will respond within 45 days of receiving a personal information request.

Personal Information Request Options

Access/Disclosure
Select this option if you wish to access what personal information Logicalis has on file about you.

Data Deletion
Select this option if you wish to have Logicalis delete any personal information we have on file on you. Logicalis will delete any information about you that is not approved by you or covered by an exemption.

File Complaint
Select this option if you wish to file a complaint regarding the use or inaccuracy any personal information we have on file for you.

Data Portability
Select this option if you wish to have a copy of your information provided to you in machine-readable format (at no cost to you).
